This questionnaire is for 5th Grade parents ONLY.  Mr. McGovern, Ms. McGrath, Mrs. Cheng and Mr. Gatzke would appreciate it if parents would fill out this form at their earliest convenience.  It will help with the organization of different activities that we have planned.  Thank you in advance for your help.

Straight Donation Drive
Every    Dollar    Makes    a    Difference    
 “Hugo Reid Annual Straight Donation Drive” 

Our PTSA spends approximately $200.00 per student per year. In order to help fund programs like the Bug Man, Character Counts assemblies, Reflections, Rolling Readers, Rocky’s Readers, Movie Nights, Intercultural Night, Book Battles, Musical Theater, Picnics, help with Field Trips, Red Ribbon Week, and special assemblies etc.,
we are asking for straight donations. 

How to donate......

Rocky’s Fundraiser Insurance - Donate $180 dollars or more per student and you will be exempt from PTSA Fundraisers (i.e. Innisbrook, First Aid Kits, See’s Candy, but NOT Parent Party/ Student Council/Fifth Grade Fundraisers) that occur throughout the rest of the 2014-2015 school year. You will also receive 1 teacher playdate raffle ticket per $180 donation to be used at the Parent Party.  
Rocky’s Balancing Act - Donate $75-$100 per student, and continue to participate in PTSA fundraisers for 2014-2015 school year and receive 5 raffle tickets per donation to be used at the Parent Party in the Spring of 2015.
Rocky’s 50/50 - Donate $50 dollars per student and continue to participate in the other PTSA 
Fundraisers offered through out the 2014-2015 school year in order for us to be able to continue to provide the programs listed above.
Your student will receive one free dress pass from Ms. Leahy.
Rocky’s Flex Plan - Donate the amount of your choice per student.
Every dollar makes a difference. 

HOW IT WORKS.... The drive begins on August 13 and ends September 12, 2014. All you 
have to do is place your donation in the envelope provided with your student’s Name and 
Teacher/classroom written on it and bring it to Raider Days and place it in the Straight 
Donation Drive Box. Should you choose to turn in your donation after Raider Days, we ask 
that you either turn it into the school office or have your child give it to his/her teacher to be 
turned in. We look forward to a community full of support!

The perks of our Straight Donation Drive..... 
• There is NO selling 
• 100% of the money goes back to the PTSA to use for our school programs listed above.
• If you work for a company who will match your donation, just contact us.
We will get you the information you need to get us more money.
• It’s a great idea to ask Aunts, Uncles, or Grandparents for a donation to your school. 
• The kids can donate too! 
• You can earn Fundraiser Insurance! 
• Each class with 100% participation will be entered into a
drawing to win a Root Beer Float party!

There will be a thermometer in the front of the school and on classroom doors to show how 
our community is doing!!

Would you like to be a published author....for the Los Angeles Times?!
The Kids' Reading Room section of the Sunday LA Times encourages submissions for Book Reviews by Kids, Jokes & Riddles, and Creativity Corner (short stories and art). You can find the Kids' Reading Room submissions written by students in the Comic Section of the paper. There have been a number of book reviews printed from Hugo Reid students in the past. 
Submissions are subject to editing. Adults should send their child's work to:
